Pre-certification Notification

This CaptionMate application is for individuals in the United States with hearing loss only. This application provides real-time captions from an automated captioning process through a federally funded program called the Internet Protocol Captioned Telephone Service (IP CTS). The registration process is mandatory for all users and must be completed prior to using CaptionMate.

You may self-certify as having hearing loss by signing the form on the following page.

After signing this page, you will have the right to withdraw consent at any time with no cost, by using the actions in your profile to withdraw consent or by contacting CaptionMate at the address provided below. You will also have the right to obtain a paper copy of the certification by mailing a check for $5 to our customer service team at:
